Has anyone tried this addin?

It allows a user parameter query against work items. I see the addin
in addin manager however, no toolbars or team menus appear. I have
restarted several times.

Just for kicks I installed VS2005 and it actually works! VS2008 does
not - same machine, same VSTS TFS Server.
